A memorial or tribute gift is a thoughtful way to honor the memory of a loved one, or recognize an individual for their achievements while making an investment in lifesaving technology, improved facilities, or new equipment at Hazel Hawkins Memorial Hospital. Gifts to the Foundation support patient care, hospital facilities, hospital equipment purchases, special programs and services, and scholarships care at Hazel Hawkins Memorial Hospital.
As a non-profit, tax-exempt organization, the Foundation receives all charitable gifts and grants in support of the Hospital. Gifts can be made in honor of or in memory of a friend, family member, or loved one. You may also designate your commemorative gift to a specific donation category. All gifts are acknowledged promptly with a thank you card sent to you for your income tax records. An acknowledgement notification is also sent to the individual being honored, or the family for whom the memorial is given.
A Naming Opportunity Program is an excellent way to establish a lasting legacy by dedicating a patient room in honor or memory of an individual, family, or a corporation. Names of donors will be recognized and prominently displayed.

Matching Gift Programs are charitable giving programs set up by corporations in which the company matches donations made by employees to eligible nonprofit organizations. For example, if a donor works for IBM and donates $1000 to Hazel Hawkins Hospital Foundation, IBM will “double the donation” by writing a check for $1000 to the same organization. Check with your employer to see if they have a Matching Program and ask for the required paperwork to make the request.
The Hazel Hawkins Hospital Foundation has established an endowment fund with the Community Foundation for San Benito County. The Community Foundation is dedicated to building a stronger community and enhancing the quality of life in San Benito County through the support of philanthropic activities. The purpose of the Endowment Program is help strengthen the nonprofit sector by creating a perpetual revenue stream generated from endowments to insure financial viability.
